The Advance Care Planning workshop provides an introduction to the Advance Care Planning Policy for Northern Ireland. It provides practical guidance on how to ensure care preferences are known in the event that a person loses their capacity to communicate for themselves. With clinical input from Foyle Hospice expertise, this session will help support professionals to hold members of their community in a sacred place that enables tender conversations about advanced illness and end of life care.
